It was a Final Jeopardy question, so the two who got it wrong wrote down the incorrect question of the two without knowing that the other had written it down.

(I was about to say, 'It was Final Jeopardy, and one of the two contestants who got it wrong did not phrase it in the form of a question.' I remember somebody actually forgetting to write down 'Where is...', but I think a few years ago, they started telling the players to write down the first part of the question first.)

Could it have been a question of the type like 'A plane crashes on the USA/Canadian border. All passengers die. Where are the survivors buried?'

Of course that one is too long to fit the box and it isn't worded as the answer, but something like that could catch players wanting to be fast on the button.

If the wrong question can be stated in different ways, and particularly if the first responder states it somewhat awkwardly, the second responder may answer with the same question stated slightly

If the answer is 'of the aardvark or the aardwolf, the one which is partially named for a pig' and the category is '8-letter words,' you could have two contestants answer 'aardwolf' and 'aardvarks,' both getting it wrong.
