So if we can make all numbers from 1-40 with these four then the maybe the question is wrong?

Obviously so, yes. Nice - I don't know how I missed that one. Probably I was assuming the question was correct and therefore didn't look very hard for the last one

you have to use ALL the numbers for each calcultion....
