The solutions already posted work for various bases:

base >= 5: On this line appears 1 0's , 4 1's, 1 2's, and 1 3's.

base >= 4: On this line appears 1 0's , 2 1's, 3 2's, and 2 3's. On this line appears 1 0's , 3 1's, 1 2's, and 3 3's.

But neither the base nor the number of digits per blank is specified in the puzzle, so here are a few more ...

base = 5: On this line appears 1 0's , 11 1's, 1 2's, and 1 3's.

base = 4: On this line appears 1 0's , 11 1's, 2 2's, and 1 3's.

And if the 'counting digits' are allowed to be in a base different than the others, then ...

base = 2: On this line appears 10 0's , 111 1's, 1 2's, and 1 3's.

Are there any solutions like this for base = 3?

You don't need that qualifier. After all, even in binary, you can refer to the symbol '2,' it just doesn't mean 'two'

Can I use leading zeroes?

On this line appears 012 0's , 012 1's, 010 2's, and 1 3's

I haven't yet thought of a solution without leading zeroes of some sort.
